This form is for agencies to provide the Commission with outcomes of investigations of serious misconduct, after a referral for action by the Commission. It does not replace a comprehensive investigation report.

Please provide the allegations that were referred to your agency, the allegations considered in the agency investigation and the outcome for each of these allegations, including any sanction or improvement action.

You will be required to upload your agency investigation report in order to submit the form. If a report was not completed, please provide a document that explains what actions were taken by your agency, the evidence considered and the outcomes reached. All decisions and actions must be capable of Commission review.
